Job summary

Connected Health Group Limited in Belfast is seeking a Community Care Manager to lead domiciliary care services across South Belfast. You will support service users and care teams, driving continuous improvement and delivering compassionate, high-quality community-based care.

You will manage a care team, handle scheduling, staff development, and client reviews, while coordinating with the Quality and Compliance teams to uphold NHS/NISCC standards and regulatory requirements.

Qualifications

• 3+ years of domiciliary care experience.
• Full, valid UK driving licence and appropriate vehicle insurance.
• Experience leading a team and creating rosters.
• Able to work in the community and cover sickness when needed.

Responsibilities

• Build and maintain relationships with the coordinator to drive growth.
• Manage on-call telephone services 7 days a week.
• Direct line management of area care team and rostering.
• Ensure staff appraisals, disciplinaries, and supervision.
• Oversee client reviews and onboarding of new clients.
• Report safeguarding and client issues to line manager.
• Coordinate with Quality and Compliance teams on NISCC.
• Maintain high-quality client care folders monthly.
