Job summary

Liverpool City Council is recruiting a Social Worker for the Preparing for Adulthood Team in Liverpool. The post supports young people with disabilities as they transition from Children's Services into Adult Social Care, working with Education, Health and Social Care colleagues to plan futures and maximise independence.

Responsibilities

• Complete strengths-based Care Act assessments and outcome-focused support plans
• Work creatively with young people, families and partner agencies to identify solutions that maximise independence and wellbeing
• Undertake Mental Capacity Assessments, Best Interest assessments, risk assessments and reviews where required
• Support young people through complex transitions, ensuring that services are coordinated and planned effectively
• Hold and manage a varied caseload, making informed and evidence-based decisions in line with legislation, policy and professional standards
• Lead and contribute to multi-disciplinary meetings with Education, Health and Social Care professionals
• Work in partnership with carers and complete Carers Assessments where appropriate
• Apply the Care Act principles of Prevent, Reduce and Delay throughout all areas of practice
• Commission and review support services to ensure individuals achieve their identified outcomes and make best use of available resources.
