Job summary

Liverpool City Council is seeking a Social Worker for the Preparing for Adulthood Team to support young people with disabilities as they transition from Children's Services to Adult Social Care. You will collaborate with Education, Health and Social Care to plan futures and promote independence.

Qualifications

• Experience completing Care Act assessments and developing outcome-focused support plans.
• Ability to undertake Mental Capacity Assessments and Best Interest assessments.
• Experience working with multi-disciplinary teams and partner agencies.

Responsibilities

• Complete strengths-based Care Act assessments and outcome-focused support plans.
• Work with young people, families and partner agencies to maximise independence and wellbeing.
• Undertake Mental Capacity Assessments and Best Interests assessments as required.
• Support young people through complex transitions with coordinated services.
• Hold and manage a varied caseload and make evidence-based decisions.
