Job summary

Sefton Metropolitan Borough Council in Sefton, UK seeks experienced Adults Social Workers to join multiple teams. Roles are available as part time or full time on an ongoing basis, with a strong focus on strengths-based, person-centred practice to support adults to maintain independence and improve quality of life.

Working across specialist Adult Social Care services, you will manage complex caseloads, complete MCA assessments and contribute to safeguarding, reviews, and interdisciplinary

Qualifications

• Experience in adult social care and safeguarding procedures.
• Ability to work in a multi-team environment across Adult Social Care.

Responsibilities

• Undertake strengths-based assessments and develop person-centred care and support plans.
• Manage a complex caseload while complying with statutory duties and legislation.
• Complete Mental Capacity Act assessments and Best Interest decisions where appropriate.
• Conduct reviews, reassessments and safeguarding enquiries in line with local procedures.
• Collaborate with service users, carers, families, health professionals and partners.
• Prepare high-quality reports and documentation, including court reports when required.
• Support adults to maximise independence, wellbeing and community participation.
• Undertake risk assessments and implement risk management strategies.
• Contribute to service improvement and development of Adult Social Care services.
