Job summary

Dorset Council is offering a fixed-term full-time Social Worker role within the Preparation for Adulthood Team in Children's Services. You will work with young people aged 14+ and families to develop independence plans, offering information, guidance and support.

The role may involve adult Care Act assessments and other related duties, with travel as required. You will join a multidisciplinary team including adults, children’s social care, education and SEN colleagues.

Qualifications

• HCPC registration is required.
• Degree in Social Work or equivalent is essential.
• Willingness to fulfil travel requirements for the post.

Responsibilities

• Complete preparation for adulthood plans with young people and families/carers.
• Provide information, advice and guidance to support the plan.
• May undertake adult Care Act assessments and related activities, including Mental Capacity Assessments/Best Interests decisions.
• Collaborate with adult and children’s social care teams and education/SEN colleagues.
