Wiltshire Council’s Locality Team (Discharge Service) is seeking a Social Worker to assess, plan and support residents with hospital-to-home transitions. You will work with healthcare professionals to ensure safe, timely discharges and promote independent living within a supportive team.

The role requires registration with Social Work England, strong communication, and knowledge of relevant legislation. We operate a hybrid model with on-site and community-based duties.

Qualificações

• Must be registered with Social Work England.
• Knowledge of safeguarding vulnerable adults and Health and Social Care legislation and policy context.
