Job summary

Connect2Halton on behalf of Halton Borough Council is seeking an experienced Social Worker to join the Hospital Discharge Team at Whiston Hospital in Prescot. You will conduct strengths-based assessments and develop personalised support plans to support timely discharges, working with adults 18+ with diverse needs.

The role requires knowledge of the Care Act 2014 and Mental Capacity Act 2005, the ability to work with multidisciplinary teams, manage your caseload, and advocate for independence

Qualifications

• Sound knowledge of social work legislation, including Care Act 2014 and Mental Capacity Act 2005.
• Experience in undertaking risk assessments and risk management.
• Commitment to strengths-based, person-centred social work.
• Excellent communication, assessment and decision-making skills.

Responsibilities

• Complete strengths-based assessments and develop personalised support plans for timely hospital discharges.
• Work with adults with a range of needs, including older people and those with disabilities.
• Promote independence, dignity and wellbeing in care decisions.
• Undertake risk assessments and contribute to risk management for discharge planning.
• Apply Care Act 2014 and Mental Capacity Act 2005 to inform decisions.
• Collaborate with multidisciplinary teams within hospital settings.
• Manage and prioritise caseload to meet deadlines.
