Job summary

Warwickshire County Council is seeking a Level 2 Social Worker to join the Hospital Social Care Team. Based at the Hospital of George Eliot Hospital, Nuneaton, and Rugby St Cross Hospital, the role involves providing a high standard social work service to older adults, managing a caseload, and supporting timely discharges across Warwickshire.

You'll undertake robust assessments and care planning, work with a multidisciplinary team, and engage with patients, families, carers and partner agencies.

Qualifications

• Must have a professional Social Work qualification and be registered with the Health and Care Professions Council (HCPC).
• Be able to work 5 days over 7 days; weekends included as part of a 4-weekly rota.
• Work closely with Multidisciplinary Health & Social Care team.
• Participate fully in team duties including meetings.

Responsibilities

• Provide a social work service to a high professional standard and manage a caseload.
• Undertake and record robust assessments and care and support planning.
• Communicate with customers, families, carers, and partner agencies.
• Travel across North and South Warwickshire as needed.
