Job summary

Leaders in Care is seeking a Post-Discharge Social Worker to support a busy transfers of care pathway, ensuring safe discharge planning and ongoing independence for service users.

You will work within a multidisciplinary team, completing assessments, arranging care reviews, and coordinating with health and social care partners. The role offers hybrid working with potential home-based days and a supportive environment.

Qualifications

• Must hold a recognised Social Work qualification.
• Registered with Social Work England.
• Experience in hospital discharge or social care preferred.

Responsibilities

• Complete post-discharge assessments within defined timeframes (14–28 days).
• Carry out 6–12 week reviews to monitor progress and adapt support.
• Use light-touch triage to determine needed interventions.
• Review care packages and coordinate further support or re-referrals.
• Collaborate with health professionals and partner agencies to achieve safe, sustainable outcomes.
• Promote independence and reduce avoidable hospital readmissions.
