Job Title: Social Worker – Learning Disabilities – Leeds

Location: Leeds

Pay Rate: £31

Liquid Personnel is currently recruiting for an Adult Social Worker to join a Learning Disabilities Team based in Leeds.

What will your responsibilities be?

• Conduct comprehensive assessments to identify the needs of individuals with learning disabilities.
• Develop, implement, and review personalized care plans in collaboration with service users, families, and other stakeholders.
• Provide advice and advocacy to ensure individuals’ rights are upheld and safeguarded.
• Work collaboratively with health, education, and social care providers to coordinate multi-agency support.
• Monitor progress, address challenges, and adjust care plans to ensure positive outcomes.
• Stay informed about relevant legislation, policies, and best practices in the field.
• Long term contract
• Regular supervision and sensible caseloads

Qualifications and Experience:

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
