Job summary

Leicestershire County Council is seeking a Community Health Reablement Worker to meet with individuals, families and carers and deliver a community-based enablement service. You will create plans to help access support, including connections with local services or referrals to other agencies, while safeguarding and assessing capacity as needed.

Visits may occur in homes, hospitals or other comfortable settings.

Qualifications

• Have health/social care qualifications to NVQ Level 2 or equivalent or experience in social care with adults.
• Experience with mental health conditions, disabilities, and working with other agencies.
• Understanding of recovery, enablement, inclusion and person-centred approaches.

Responsibilities

• Meet individuals, families and carers to connect and listen and deliver enablement support.
• Devise plans to access local services, refer to agencies or provide needed support.
• Identify safeguarding risks and report, assess capacity, and ensure safety.
• Undertake visits to clients in home, hospital or other preferred locations.
• Work collaboratively with children’s services, education and health, and travel across the county.
