• Expert knowledge of relevant legislation, social work theory, policy and practice.
• Experience in strengths-based social care approaches.
• Ability to assess complex needs, risks, and strengths.
• Strong written and verbal communication skills.
• Ability to collaborate in multi‑disciplinary teams.

Responsibilities

• Work in multi-disciplinary teams across Cornwall to deliver adult social care.
• Lead discharge planning and wrap‑around support to help people remain at home and in the community.
• Promote person‑centred care and independence, respecting the wishes of service users.
• Complete comprehensive assessments and coordinate with partners to arrange services.
