Devon County Council’s Integrated Adult Social Care Team in Exeter West is seeking passionate Social Workers. You will use a strengths-based, person-centred approach to support adults to maximise independence and choice, and complete complex assessments.

You will work with health partners and multi-disciplinary teams, mentor students and less experienced colleagues, contribute to service development, and benefit from flexible, hybrid working and a supportive culture.

Formación

• Registered Social Worker with Social Work England.
• Experience in adult social care practice, including risk assessment and safeguarding.
• Knowledgeable about Care Act, Mental Capacity Act and Mental Health Act.

Responsabilidades

• Use strengths-based, person-centred approach to maximise independence and choice for adults.
• Complete complex assessments using health and social care tools, including Care Act assessments.
• Apply the Mental Capacity Act, including two-stage test and Best Interest decisions.
• Identify and promote enablement and reablement opportunities.
• Lead safeguarding inquiries and work with multi-disciplinary teams.
• Mentor students, newly qualified social workers and less-experienced colleagues.
