Our team operates across the High Peak area of Derbyshire using a flexible and largely agile approach to working. We utilise link worker and champion roles to support the delivery of Care Act 2014 assessments and reviews through both face-to-face and telephone-based interventions. As a Social Worker within our team, you will be expected to demonstrate a high level of autonomy, professional accountability and effective case management. Timely recording of interventions and the ability to progress work efficiently are essential. You will work closely with a range of partner agencies including Health services, the Police, Housing providers and private, voluntary and independent sector organisations. Building positive professional relationships, identifying opportunities for signposting and supporting carers are key elements of the role. We are committed to promoting independence, wellbeing and person-centred, strengths-based outcomes for the people we support.
The role also includes participation in Duty cover. You will need to respond effectively to changing circumstances, triage incoming work and assess urgency to ensure adults and carers receive the appropriate level of support.

As this post involves working with vulnerable adults, the successful candidate will be required to undertake a Disclosure and Barring Service (DBS) check.
