About the Role

This is a rewarding opportunity to join a private organisation delivering high-quality, personalised support to adults facing a range of social and emotional challenges – including mental health, isolation, life transitions, or safeguarding issues.

Support is provided through a flexible mix of face-to-face sessions, as well as remote contact via telephone and video link, allowing for responsive and accessible care tailored to each individual’s needs.

Key Responsibilities

• Carry out person-centred assessments and develop individual support plans
• Provide one-to-one emotional and therapeutic support
• Deliver interventions in-person and remotely (telephone and video)
• Monitor outcomes, manage risk, and follow safeguarding procedures
• Liaise with families, carers, and external professionals to coordinate care
• Maintain accurate and confidential case records
• Contribute to ongoing service improvement

About You

• Degree or equivalent qualification in Social Work
• Registered with the appropriate UK regulatory body (e.g. Social Work England)
• Strong interpersonal and communication skills
• Counselling skills or therapeutic experience (desirable)
• Confident working independently and remotely
