This is an exciting opportunity for an experienced Learning Disability Social Worker to make a real difference in the lives of individuals in Hammersmith and Fulham. We are looking for a skilled professional to join our dedicated team and provide outstanding support to adults with learning disabilities and their families, in accordance with the Care Act 2014. This full-time role offers an ongoing contract and a competitive pay rate of £32 per hour.

What you will do:
Provide a professional social work service to adults, their families, and carers in line with Care Act 2014 requirements.
Engage with individuals, families, groups, and communities to assess needs and employ a range of interventions promoting choice, control, and independence.
Demonstrate expert and effective practice in complex situations, managing higher levels of risk while liaising with a wide range of professionals.
Assist individuals, carers, and families in developing support plans using a strength and asset-based approach.
Actively promote wellbeing and independence and facilitate contingency planning to address complexities and changing circumstances.
Provide guidance on the use of Personal Budgets and Direct Payments to enable informed decisions.
Ensure compliance with relevant legislations, including the Care Act 2014, when using advocacy services and carrying out reviews.
Promote a learning culture and provide professional reflective supervision to less experienced Social Workers.
Train as a Practice Educator or Approved Mental Capacity Professional to support statutory duties under the Liberty Protection Safeguards Scheme.
