Rate: Up to £35 per hour

An exciting opportunity has arisen at the North London Local Authority for a Social Worker to join their Hospital Discharge Team.

Responsibilities:
  • Conduct strengths-based, person-centered assessments
  • Manage a varied caseload, including capacity assessments, best interest decision-making, and Care Act assessments
  • Participate in a duty rota
  • Deliver exceptional social care to adults and their carers, promoting choice and control
  • Engage in outcomes-focused and preventative conversations to build on individuals' strengths and local networks
Qualifications and Experience:
  • Degree in Social Work
  • Registered with the SSSC
  • Knowledge of relevant legislation and policies
  • Excellent communication and interpersonal skills
