Job Title: Adult Senior Practitioner – Community Independence and Wellbeing Team
Location: London Borough of Hammersmith and Fulham
Rate: £35 per hour

An exciting opportunity has recently become available for an Adult Senior Practitioner. They are looking for a new member to join the Community Independence and Wellbeing Team in the London Borough of Hammersmith and Fulham.

Responsibilities include:

  • Conducting comprehensive assessments of adults in the community to determine their needs and develop personalized care plans.
  • Providing ongoing support and intervention to ensure the well-being and safety of vulnerable adults.
  • Collaborating with multidisciplinary teams and external agencies to coordinate services and resources for clients.
