Job title: Mental Health Unit / West Mental Health Social Work Team – Social Worker (PO1) / Newly Qualified (SO1)

Contract Type:

Permanent, 37 hours per week

About Us:

We are a dedicated and compassionate social work team based within the West Community Mental Health Team. We are based over two office bases, Millfield House and St Mary’s Hospital. We are committed to providing a high-quality service to individuals with severe and enduring mental health needs. We work across Working Age Adults Mental Health Services and Older People’s Services, supporting discharge from hospitals and working collaboratively with health colleagues and other agencies to improve the lives of service users, their families, and carers.

Job Description:

We are seeking an empathetic Social Worker to join the West Community Mental Health Team. Responsibilities include managing caseloads, conducting strength-based assessments, undertaking Care Act assessments, and developing personalized care plans within legal frameworks such as the Human Rights Act, the Care Act, the Mental Capacity Act, and the Mental Health Act. The role involves safeguarding, risk assessment, report compilation, and collaboration with service users and professionals to ensure holistic, person-centered care. A commitment to ongoing professional development and interest in post-qualifying roles like Approved Mental Health Professional, Best Interest Assessor, and Practice Educator is essential.

Key Responsibilities:
  1. Conduct strength-based, person-centered assessments involving carers, families, and advocacy services.
  2. Develop and implement personalized care and support plans with choice and control.
  3. Collaborate with healthcare professionals, agencies, and support networks for integrated care.
  4. Advocate for clients’ rights and access to services.
  5. Maintain accurate case records and documentation.
  6. Participate in team meetings and professional development activities.
  7. Use safeguarding frameworks for risk management.
  8. Prepare reports for tribunals and hearings based on complex evidence.
  9. Work closely with clients, families, carers, and healthcare professionals to ensure holistic care.
  10. Demonstrate anti-oppressive practice aligned with social work values.
  11. Engage in continuous professional development, including post-qualifying opportunities.
Qualifications:
  • Bachelor's or Master's degree in Social Work / Social Work qualification.
  • Registered with Social Work England.
  • Strong interpersonal and communication skills.
  • Experience working with individuals with mental health needs.
  • Ability to work independently and in a team.
  • Commitment to ongoing professional development.
