Job summary

Farmfield Hospital in Charlwood, Surrey is seeking a dedicated Social Worker to join a multidisciplinary team. You will manage a complex caseload, conduct social care and risk assessments, facilitate group activities and support service users in planning their futures.

You will report to the Safeguarding Lead, handle internal investigations and liaise with home authorities during admission and discharge, while benefiting from ongoing training and career development opportunities within a

• A Social Work degree is required.
• Registration with Social Work England is required.
• Experience within a similar environment is preferred.
• Experience working with vulnerable adults is desirable.

Responsibilities

• Referrals to LADO and Local Authority in line with safeguarding and assessments.
• Educating hospital staff about safeguarding matters.
• Completing annual social care reassessment within the CPA format.
• Facilitating access to welfare rights, housing, advocacy and related support services.
• Participating in 1:1 supervision.
