Job summary

Elysium Healthcare Limited seeks a Social Worker for Farmfield Hospital in Charlwood, Surrey. You will manage complex caseloads, perform social care and risk assessments, and support service users to plan for their future within a multidisciplinary team.

You will liaise with safeguarding authorities, contribute to ICR meetings, and help service users transition during admission and discharge, with access to ongoing training and development opportunities.

Qualifications

• A Social Work degree.
• Registration with Social Work England.
• Experience within similar environment.
• Experience with working with vulnerable adults.

Responsibilities

• Referrals to LADO and Local Authority in line with safeguarding and assessments.
• Educating Hospital staff about relevant safeguarding matters.
• Completing a full annual social care reassessment within the CPA format.
• Facilitating access to welfare rights, housing, advocacy services and associated support services.
• Participating in 1:1 supervision.

As a Social Worker you will manage a complex caseload and be responsible for social care assessments, risk assessments, facilitate group activities, and will contribute to ICR meetings, supporting service users to make viable plans for their future. Y ou report to the Safeguarding Lead, and will manage internal investigations and liaise with home authorities for service users during their admission and discharging process, while enjoying access to a range of training opportunities that can see your career grow and you achieve your career aspirations. Where you will be working

Location: Farmfield Drive, Charlwood, Surrey, RH6 0BN

You will be working at Farmfield Hospital, situated located next to Gatwick. You'll be caring for adult men 18+ who potentially have a forensic background and enduring mental health illnesses such as personality disorder, substance misuse issues and challenging behaviours.
