Job summary

Milton Keynes City Council is recruiting a Social Work Assistant to join the Assessment, Review and Hospital Discharge Team. You will support adults to access the right support, carry out assessments, reviews and care plans, and help people transition from hospital to community.

This site-based role requires a driving licence, collaboration with carers and health professionals, and commitment to safeguarding.

Qualifications

• Ideally educated to Level 3 in Health and Social Care, or hold an equivalent qualification, or be able to demonstrate significant relevant experience and practical knowledge.
• An effective communicator who can build positive relationships with adults, carers, families and professionals from different services.
• Someone who understands the importance of treating people with dignity, respect and empathy at all times.
• Confident using information technology systems to maintain accurate records and communicate effectively.
• Able to organise competing priorities while maintaining a high standard of service and attention to detail.
• Committed to personal development and working in line with our values of being Dedicated, Respectful and Collaborative.

Responsibilities

• Recognise safeguarding concerns and potential risks, taking appropriate action and escalating issues when required.
• Support the completion of assessments, reviews and care plans that focus on strengths, independence and positive outcomes.
• Manage a caseload with guidance and record interventions accurately using council systems within agreed timescales.
• Work closely with individuals, carers, families and partner organisations to coordinate support and achieve agreed outcomes.
• Provide practical support where required to help people maintain their independence and wellbeing.
• Review progress with support from supervision and contribute to improving outcomes for the people you work with.
