Job summary

Milton Keynes City Council is hiring two Senior Social Workers to join our Adult Social Care, Mental Health & Complex Needs team. This site-based role offers opportunities to work with experienced managers and frontline social workers to support adults with mental health needs and complex circumstances.

You will undertake strengths-based assessments, develop support plans, manage complex cases and collaborate with health services and partners to promote independence and wellbeing.

Responsibilities

• Undertake strengths-based assessments to determine need and outcomes.
• Develop and monitor support plans for adults with mental health needs and complex circumstances.
• Manage complex cases and coordinate services with partners to promote independence and safety.
• Work collaboratively with a range of professionals to deliver care under the Care Act and Mental Health legislation.
