Job summary

Milton Keynes City Council is recruiting a Social Worker to join the Adult Services- Mental Health team in Milton Keynes. You will undertake strengths-based assessments, develop support plans, and coordinate services to promote independence and wellbeing for adults with mental health needs and complex circumstances.

The role is site-based with potential for some home working; interviews are scheduled and a retention payment may apply.

Qualifications

• Qualified Social Worker registered with Social Work England.
• Strong strengths-based practice with adults who have mental health needs and complex circumstances.
• Experience in assessment, risk management and case coordination.

Responsibilities

• Lead and coordinate complex cases with high risk, including safeguarding under Care Act sections.
• Undertake strengths-based assessments and develop outcome-focused support plans.
• Collaborate with families, health professionals and partner organisations to achieve positive outcomes.
