Job summary

Milton Keynes City Council is recruiting a Social Work Assistant to join the Assessment, Review and Hospital Discharge Team. The role focuses on supporting adults at key points in their lives, helping them access the right support and remain independent, with safe hospital discharges.

You will work with adults, carers, health professionals and partner organisations to assess needs, review support and help people transition back to their communities.

Qualifications

• Ideally educated to Level 3 in Health and Social Care, or equivalent qualification, or able to demonstrate significant relevant experience.
• An effective communicator who can build positive relationships with adults, carers, families and professionals.
• Able to treat people with dignity, respect and empathy at all times.
• Confident using information technology systems to maintain accurate records and communicate effectively.
• Able to organise competing priorities while maintaining a high standard of service and attention to detail.

Responsibilities

• Recognise safeguarding concerns and escalate issues when required.
• Support the completion of assessments, reviews and care plans focused on strengths and independence.
• Manage a caseload with guidance and record interventions within timescales.
• Coordinate support with individuals, carers, families and partner organisations.
• Provide practical support to help people maintain independence and wellbeing.
• Review progress with supervision and contribute to better outcomes for residents.
