Job summary

Cardiff Council invites applications for a Grade 6 Social Work Assistant within the Community Duty Team. You will work with Social Workers, Occupational Therapists and partners to provide person-centred support for adults across Cardiff.

You will gather information, assist with assessments and reviews, and help people access services, while maintaining clear records and contributing to service improvements in a busy environment.

Qualifications

• Experience in health and social care or related setting is desirable.
• Excellent communication and interpersonal skills are required.
• Ability to gather and record information accurately and maintain case records.
• Good IT skills and ability to use standard software for reporting and documentation.

Responsibilities

• Support Social Workers in delivering person-centred services.
• Assist with assessments, support planning, reviews and interventions.
• Gather information from individuals, families and carers and help them access services.
