Job summary

Bridgend County Borough Council is seeking a Social Work Assistant to join the Early Intervention and Prevention Hub. The role involves managing a 37-hour week and working within a strengths-based approach to provide information, advice and access to universal services for adults whose independence is affected.

You will support case coordination, carry out assessments under supervision, and collaborate with professionals to deliver short-term enabling care and support.

Qualifications

• Carrying a delegated caseload under supervision in line with your skills and experience.
• Ability to work collaboratively with social workers and other professionals on cases.
• Commitment to safeguarding and Welsh language considerations as part of care and support.

Responsibilities

• Hold and manage a delegated caseload under supervision.
• Collaborate with a social worker or other professionals on specific cases as needed.
• Carry out initial, time-specific assessments and arrange appropriate care and support.
• Work with service users and carers to ensure outcomes in care plans are met.
• Maintain accurate records in line with procedures and participate in supervision and appraisals.
