Job Title: Social Care Worker

We are seeking to appoint a Social Care Worker with experience in adult social care. The East Locality Team covers the east of Middlesbrough.

The service promotes flexible working for all staff, including agile working from home and Middlesbrough Council bases. A blended working contract is offered.

Responsibilities:

1. Assessing individuals, including service users and carers, living in east Middlesbrough.
2. Completing comprehensive care needs assessments and support plans.
3. Implementing services for vulnerable individuals experiencing trauma.
