Job summary

East Riding of Yorkshire Council is seeking a Social Care Coordinator to join the Bridlington Adult Community well-being team. This fixed term 12-month role offers a blended office/remote working pattern to support adults over 18 to remain independent in their communities.

You will assess needs, build networks, and promote independence through strength-based approaches. Travel between locations is expected and a full driving licence is essential.

Qualifications

• Qualified and resilient social care professional with relevant accreditation.
• Ability to work across organisations and teams.
• Clear, compassionate communication.
• Understanding of Care Act 2014, Mental Capacity Act 2005, Human Rights Act.
