Job summary

Reading Borough Council is seeking a Social Care Coordinator to support adults with long-term needs within the Physical Disability and Older People Team. You will undertake assessments, contribute to support planning, and coordinate care with families and providers to secure timely, quality outcomes.

The role expects experience in health or social care, a strong understanding of the Care Act, and a commitment to safeguarding.

Qualifications

• Experience in health or social care settings benefiting care outcomes.
• Good understanding of the Care Act and adult safeguarding principles.
• Ability to assess needs and support planning in a timely manner.

Responsibilities

• Completing Care Act assessments and reviews under qualified social workers guidance.
• Developing and implementing person-centred support plans.
• Coordinating care packages and liaising with providers and external agencies.
• Promoting independence and wellbeing through a strengths-based approach.
• Maintaining accurate case records in line with statutory requirements.
