Qualifications

Job description

Service Support Worker (Home from Hospital Service) - Croydon, London

Salary: £22,932 (plus Inner London Weighting £3,344 if applicable) per annum pro rata

Hours: 28 per week

Contract: Fixed Term (until end of June 2026 with possibility to extend)

Driving: A full driving licence and access to a vehicle is preferred but not mandatory

What will a day in the life of a Support Worker involve?
  • Providing practical and emotional assistance to people returning from hospital or medical facilities.
  • Accompanying service users to attend hospital appointments and other activities.
  • Liaising with health, housing and other professionals to ensure that the needs of the service user are being met.
  • Ensuring a safe living environment for service users by undertaking risk assessments.
  • Carrying out welfare checks to assist service users with daily activities.
  • Building positive relationships with service users, their families, and healthcare professionals.
  • Ensuring all care documentation is accurate and up to date.
What does it take to be a successful Support Worker?
  • Previous experience in a similar care setting, preferably within healthcare or social care.
  • Compassionate and empathetic nature, with a strong desire to make a positive impact.
  • Excellent communication and interpersonal skills, effective within a multidisciplinary team.
  • Knowledge of person-centred care principles, upholding dignity and autonomy.
  • Ability to work independently, demonstrating initiative and reliability.
  • Flexibility to work various shifts, including evenings, weekends, and bank holidays.
  • A full driving license and access to a vehicle is preferred but not mandatory.
