Job summary

Worcestershire County Council is seeking a Service Assessor to deliver initial community assessments, identify needs and promote independence. The post is permanent, based in Worcestershire, full-time 37 hours per week.

You will work within the Reablement team, develop Promoting Independence Plans, case manage service users, and participate in quality assurance. A Level 3 Diploma in Health and Social Care or relevant experience is required. Salary £31,537 - £33,695 per annum.

Qualifications

  • Initial community assessments to identify needs, risks and independence potential.
  • Develop Promoting Independence Plans detailing required interventions.
  • Active case management within a locality as part of a multidisciplinary team.
  • Adhere to standards and performance measures including monitoring protocols and QA audits.
  • Provide out of hours cover as part of a rota.

We will make sure people receive the right support at the right time to help them live independently at home for as long as possible. This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life and remain independent for as long as possible. We are looking to recruit to the position of Service Assessor, you must be committed to:

As the ideal candidate you must have a level 3 Diploma in Health and Social Care or considerable experience of working in a social care setting. It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate and empathetic.

In return you will be supported in your role by your line manager through one to one meetings and performance reviews, team meetings and other corporate wellbeing tools. You will be encouraged and supported to complete any formal accredited qualifications, and you will have a personal development plan to support you in developing towards any personal career aspirations.

This post is exempt under the Rehabilitation of Offenders Act 1974 and as such appointment will be conditional upon the receipt of a satisfactory response to a check of criminal records via the Disclosure and Barring Service before the appointment is confirmed. This will include details of cautions, reprimands or final warnings, as well as convictions.
