Job summary

Birmingham Community Healthcare NHS Foundation Trust is seeking a Band 6 Virtual Ward Practitioner to provide clinical expertise and operational leadership for Hospital @Home across Birmingham. The post focuses on identifying and preventing avoidable admissions and facilitating early discharge within MDT collaborations.

The role involves direct patient care, supervision, and complex adult caseload management in diverse settings, including community and secondary care, with flexible working

Qualifications

Evidence-based clinical practice in adult care.
Ability to work across community and hospital settings.
Understand NMC/HCPC codes and safeguarding.

Responsibilities

Provide clinical care and expert support to Virtual Ward model.
Collaborate with MDTs across Secondary and Primary Care.
Assess, diagnose, plan treatment, and discharge patients.
Support safe, timely discharge and patient safety using digital platforms.
Attend shifts 08:00–18:00; flexibility for rota.
