Job summary

Birmingham Community Healthcare NHS Foundation Trust is seeking a dedicated nurse/therapist to work within a multidisciplinary team. You will contribute to integrated care pathways, promote evidence-based practice, and support patients and relatives to make informed health choices.

You will participate in MDTs across secondary care, inpatient, UCR and community services, facilitate teaching, supervise junior staff, ensure safe discharge, and use digital platforms for patient monitoring.

Work in accordance with the NMC / HCPC Code of Practice Be conversant with all BCHC NHS FT policies and procedures and ensure they are correctly implemented. Ensure evidence-based care is practiced and promoted at all times Manage own time effectively and prioritise workload Facilitate teaching and supervision for less experienced members of staff. Promote health by collaborative working with patients and their relatives. Provide support to the multidisciplinary team to enable patients to make better choices for their health and that of their families, undertaking health promotion and contributing to the prevention of adverse effects to health and wellbeing. Act as a role model to and facilitate others to respect the dignity, wishes and beliefs of patients: involving them in shared decision making and obtaining consent prior to all interventions. Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse and acting upon concerns immediately. Contribute to the review and development of integrated care pathways and clinical guidelines to ensure a standardised approach to care Participate in the further development of an integrated, multi-disciplinary EICT and Virtual Ward working alongside colleagues in Integrated MDT;s and the wider health and social care economy to facilitate achievement of patient outcomes. Triage and co-ordinate other junior members of the team and appropriately delegate specific aspects of nursing and therapy care according to the care plan. Participate in MDTs within Secondary Care, In-patient, UCR and Community Services. facilitating the continuity of patient care ensuring patients and relatives are kept informed of treatments, progress, and discharge arrangements. Have responsibility for the safe use of highly complex patient monitoring and support equipment within the work area Communicate effectively verbally and in writing with colleagues and stakeholders, utilising electronic methods of communication within the remit of the Data Protection, ensuring clinical information is conveyed within a timeframe that ensures patient safety. Fully utilise digital platforms, patient systems and IT to support patient monitoring and clinical care delivery.Facilitate safe discharge from caseloads to an appropriate community setting.
