Job summary

The Birmingham Community Health Care NHS Foundation Trust is seeking a clinician to provide clinical expertise and operational management for the Virtual Wards (Hospital @Home) model across Birmingham. The post involves working within city-wide settings, in-reach into secondary care, and collaboration with Primary Care and Urgent Care services to prevent admissions and enable early discharge.

Responsibilities

• Provide clinical expertise and operational management for the Virtual Wards (Hospital @Home) model.
• Work across city-wide settings, including in-reach into secondary care to prevent avoidable admissions.
• Collaborate with Secondary and Primary Care, Urgent Care Response, Early Intervention Team and Specialists Services.
• Attend MDTs to provide advice and support and supply data to support the model.
• Assess, diagnose and manage a highly specialist adult patient caseload in home and community settings.
• Discharge planning and facilitating early discharge where appropriate.
