Job summary

Birmingham Community Healthcare NHS Foundation Trust is recruiting a Service Manager – Intermediate Care Group to lead the Virtual Ward (Hospital @Home) model. You will work across city-wide settings with MDTs to prevent hospital admissions and facilitate early discharge, including in community and secondary care settings.

The role requires advanced clinical leadership, robust communication, safeguarding knowledge, and willingness to work across multiple sites.

Qualifications

• Significant experience as a registered health professional including community/frailty settings.
• Experience in multidisciplinary approach to care within community services.
• Up-to-date post-registration education relevant to the role.
• Understanding of safeguarding and clinical governance requirements.

Responsibilities

• Work with MDTs to provide expert clinical advice and support.
• Assess, plan and manage care for a highly specialist adult caseload at home.
• Coordinate with Secondary and Primary Care to prevent admissions and enable early discharge.
• Ensure evidence-based care and effective use of digital patient systems.
• Promote health in partnership with patients, relatives and the MDT.
• Support teaching and supervision of less experienced staff and supervise caseloads.
