Job summary

Birmingham Community Healthcare NHS Foundation Trust in Birmingham is seeking a Band 6 Virtual Ward Practitioner to deliver clinical excellence within the Hospital @Home model. You will work across hospital and community settings to identify, assess and manage patients at risk of admission, enabling safe, timely discharge where appropriate.

You will join integrated MDTs, provide expert clinical guidance, and support operational delivery of the Virtual Ward in collaboration with Urgent Care

Qualifications

• Experience in community health/nurse practitioner roles.
• Knowledge of MDT working and hospital-at-home models.
• Strong clinical decision-making and leadership skills.

Responsibilities

• Assess, diagnose, and manage adult patient caseload in home and community settings.
• Provide expert clinical support and advice within teams.
• Contribute to integrated care pathways and discharge planning.
• Promote safe discharge and prevention of unnecessary admissions.
