Job summary

B. Braun Medical Ltd. is seeking a dedicated Clinical Homecare Nurse to join our team in South Wales, covering Cardiff and surrounding areas.

The role involves delivering parenteral nutrition and IV therapies in patients' homes, with NHS collaboration and patient-focused education. The ideal candidate is a Registered Nurse with strong communication skills, capable of working around a 40-hour rolling rota, and willing to travel to support homecare patients.

Qualifications

• Registered General Nurse with current NMC qualifications.
• Mentoring/Teaching Certificate or equivalent (or attainment within 12 months).
• Excellent communication and time management skills; able to work under strict timescales.
• Ability to make correct clinical decisions in emergencies.

Responsibilities

• Administer prescribed IV therapies and provide care to patients.

• Assist in disconnections/ reconnects of PN and IV therapies for independence.

This position provides the successful candidate with the opportunity to enhance professional growth whilst developing existing skills.

Location: Working with trusts and patients inSouth Wales and the surrounding areas.

Working hours: Monday toSunday, 40 hours over 5days per week on a rolling rota.

The shifts are structured as split shifts to accommodate patient needs. This means you will typically work around 4 hours in the morning to disconnect patients, and another 4 hours in the evening to reconnect them, with a break in the middle of the day. Flexibility is essential; some days may be longer or shorter depending on patient visits, but overall, average out to 40 hours per week.

The salary for the role is £43,391 per annum.

Your main responsibilities:

• To administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
• Provide training, education and support to patients/carers with regards to the equipment and infusion devices required for infusion/administration of therapies to independence.
• Provide training, education and support to patients/carers in the connection and disconnection of PN and IV therapies to independence.
• Provide training, education and support patients/carers in the care and management of their CVAD in accordance with local policies and national guidelines to independence.
• Participate on the on-call rota, make autonomous decisions to resolve out of hours queries and elevate to the Senior on-call if required.
• Work with the Practice Development Nurse and Regional Lead Nurse in discharge management including pre-discharge assessment visits and home assessments and following relevant processes, to provide a seamless service.
• Support the Installation of new TransCare patients – complete all relevant documentation and elevate any issues.

Experience Required:

• Intensive Care / High dependency / General surgery/nursing care experienceat a level equal to top of Band 5.
• Current clinical experience minimum of last 2 years.
• Competent in the management and delivery of PN and other IV Therapies.
• Competent in the care and management of CVADs.
• Blood samples from CVADs.

What you need to succeed:

• A Registered General Nurse with current NMC qualifications.
• Mentoring/Teaching Certificate or equivalent (or attain within 12 months).
• Excellent communication skills, time management skills and the ability to work well against strict timescales.
• The ability to make correct clinical decisions in a clinical emergency situation.
• Flexibility to travel to support Homecare patients and/or careers is essential.
