Job summary

Active Care Group is seeking a Senior Support Worker for Russell Hill in Purley to join a supportive team. You will provide direct care to adults with autism and complex needs, working within a multidisciplinary group and contributing to care planning and delivery.

The role involves completing clinical tasks as directed, maintaining thorough records and supporting residents to achieve their goals. NVQ Level 3+ in Health & Social Care is desirable.

Qualifications

• A genuine interest in providing person-centred care.
• NVQ in Health and Social Care Level 3+ desirable.
• Willingness to work across shifts including weekends.

Responsibilities

• Provide direct care and support as part of a multidisciplinary team.
• Assist the nursing team with assessment, planning, delivery and evaluation of care.
• Act as a key team member to help residents achieve their planned outcomes.
• Undertake clinical tasks as delegated by senior nursing staff after training.
• Maintain accurate, legible records of care based on the care plan.
• Develop and maintain good working relationships with residents, families and professionals.
