Job summary

Lyndale, part of Keys Group in Hereford, is seeking a Senior Support Worker to lead a small team in delivering person-centred care for up to 10 adults with mental health needs, learning disabilities or autism.

You will manage Support Workers, follow care plans, report records, liaise with health professionals and families, and help ensure a safe, homely environment for all residents.

Qualifications

• NVQ / QCF Level 3 in Health & Social Care is required.
• Experience in health and social care activities may be beneficial.

Responsibilities

• Lead and support a team of Support Workers.
• Deliver high standards of care in line with care plans.
• Maintain records and report accurately.
• Liaise with health professionals and families.
• Contribute to a safe, homely environment.

About Lyndale

Lyndale is a residential care home in Hereford supporting up to 10 adults with mental health needs, learning disabilities or autism.

As a Senior Support Worker, you will be involved in the day-to-day support and routine of the people within the service including:

• Managing a team of Support Workers
• Providing high standards of care and support for people we support, in line with care and support plans
• Encourage good community relationships by promoting a variety of activities
• Accurately report, record and maintain records as required and working to care plans
• Liaise with health professionals and their families
• Complete supervisions and audits
• To ensure that all duties undertaken by reporting staff are performed to required standards, and that all such staff contribute in a positive manner to provide a happy, efficient and homely atmosphere within the Home.
