You will collaborate with Recovery Coordinators and clinical teams to ensure seamless, responsive care. Permanent contract with full-time hours (37.5 per week) and a pro rata salary depending on hours worked.

• Manage a complex caseload and provide tailored, practical support.
• Deliver a range of interventions and help people engage with health and treatment services.
• Work closely with Recovery Coordinators, clinical colleagues and partner agencies to ensure care is joined up and responsive.
