Responsibilities

• Provide expert assessment, clinical reasoning and evidence-based intervention for people with complex and multifactorial needs.
• Manage a complex clinical caseload including triage, holistic assessment, care planning, intervention and ongoing review.
• Support admission avoidance and timely hospital discharge and rehabilitation in the patient’s preferred place of residence.
• Lead and support implementation of evidence-based clinical standards and contribute to quality improvement.
