Job summary

mbi Medical Ltd is seeking an experienced practitioner to undertake assessments under the Community Care Act using self directed support to tailor care plans. You will coordinate and monitor packages, review outcomes, and advocate for service users and carers where needed.

You will lead safeguarding investigations, manage highly complex risk, and prepare written reports for court proceedings and case conferences.

Responsibilities

• Undertake assessments under the Community Care Act and related legislation using a self directed support approach.
• Coordinate and monitor care packages and support plans; review outcomes and advocate for service users or carers as needed.
• Engage with individuals to assess risk and support decision making about functioning in their community.
• Lead Safeguarding Adults investigations following the local No Secrets multi‑agency policy.
• Promote safeguarding awareness with service users and colleagues; stay updated on current guidance.
• Take a lead in complex cases involving safeguarding issues, family dynamics, conflict or challenging situations.
• Provide written reports for court proceedings, case conferences and other forums.
• Work with highly complex risk, including capacity and best interest considerations.
• Engage in inter-disciplinary relationships and partnerships to support integrated working.
• Operate according to NCC and departmental procedures and maintain electronic records (CareFirst).
• Provide practice guidance to less experienced staff under supervision.
• Supervise competency-based staff and students as appropriate.
• Ensure staff record and maintain quality information and data in line with departmental requirements.
• Promote service access across diverse ethnic, religious, cultural and linguistic backgrounds and challenge where needed.
• Maintain awareness of department resources and advocate for access to health resources.
• Encourage development of resources and partnerships to benefit service users and maintain positive inter-agency relationships.
• Familiarise partner colleagues with department tasks and responsibilities and participate in appropriate inter‑agency training.
