mbi Medical Ltd in the United Kingdom is seeking a qualified professional to undertake the full Care Management process under the Care Act. You will maintain comprehensive case records, carry out risk assessments, and contribute to safeguarding and quality assurance efforts.

The role involves training to become or maintain Best Interest Assessor status, working with health and statutory partners, and supporting DoLS duties and joint working across agencies.

Responsabilités

• Carry out range of activities for the Care Management process as outlined in the Care Act.
• Maintain up-to-date paper and IT-based case records.
• Identify risk areas and complete risk assessments following Merton procedures.
• Be or become Best Interest Assessor to support DoLS duties.
• Safeguard adults through safeguarding personal approaches and quality assurance.
• Ensure commissioned services represent best value and meet identified needs.
• Provide statistical information and co-operate in research with the Team/Service Manager.
• Maintain reports, mileage, hours, and daily diary records.
• Input data into CareFirst accurately and timely.
• Attend meetings, courts, tribunals, and hearings as required.
• Develop skills through training with Team Manager/Service Manager.
• Work under supervision of Team/Assistant Team Manager and maintain professional standards.
• Engage in joint working with partner agencies, particularly Health staff.
• Practice under Self Directed Support and Mental Capacity Act 2005.
• Carry out safeguarding enquiries per multi-agency protocol.
• Participate in a duty system for known customers and direct access to the team.
