Job summary

mbi Medical Ltd seeks an experienced social worker to deliver person-centered care, undertake comprehensive assessments, and manage designated caseloads within the Dorset Health Services framework.

The role requires engagement with families, safeguarding duties, and attendance at court or statutory meetings as needed. Training in AMHP and related duties is expected or supported. A commitment to quality and CPD is essential.

Responsibilities

• Carry out social work tasks to a satisfactory professional standard in accordance with the policies of the Social Services Committee and instructions of the Service Unit Head.
• Account to the Service Manager for the quality of work and provide information on the nature of the work.
• Ensure that support plan reviews are carried out in a timely and person centred way evidencing need for continuing support.
• Undertake comprehensive assessments and contribute to risk assessment and risk management plans for adults.
• Undertake care coordinator role for designated caseload in line with policy and procedures.
• Undertake carers assessments and work with families.
• Provide information and advice to adults and their carers to help people stay healthy and plan for future needs.
• Encourage and support people to look after themselves and promote choice and control for people and use of personal budgets and direct payments where possible.
• Undertake best interest assessments under the Mental Capacity Act.
• Assist adults to have realistic expectations about the support the Council can offer through involvement and communication.
• Act as an Approved Mental Health Professional (or undertake training) and make admissions to hospital under the Mental Health Act.
• Act as a Safeguarding Adult Practitioner (or undertake training).
• Train and act as a Deprivation of Liberties Assessor.
• Act as Care Manager as required and deliver the service in accordance with statutory responsibilities.
• Maintain essential records for statistical purposes and make information available as required.
• Participate in team meetings, Multi-Agency Risk Meetings, Child Protection Meetings, Core Group Meetings and Safeguarding Meetings.
• Participate in out-of-office hours standby and statutory functions with appropriate preparation.
• Contribute to training and provide recovery focused interventions.
• Provide a Team or Service Unit consultancy service as required.
• Undertake development roles in the Team or Service Unit.
• Engage in supervision and appraisal and maintain CPD.
• Prepare reports for court and attend as required.
• Perform liaison roles and undertake additional duties commensurate with the post.
