Job summary

The Vale of Glamorgan Council is seeking a highly motivated social worker to undertake care and support assessments, including CHC assessments, for adults with complex or ongoing needs. You will assess and support service users and carers, applying relevant legislation and safeguarding practices.

Successful applicants will have a Social Work degree and be registered with Social Care Wales, with experience in adult services.

Qualifications

Experience of working with adults who are vulnerable through illness, disability or age related frailty.
Assessment of Adult service users and carers.
Knowledge of relevant legislation e.g. Social Services and Well-being (Wales) Act 2014, Mental Health Measure (Wales) 2010, Mental Capacity Act 2005 and Mental Health Act 1983.
Knowledge of Safeguarding issues and procedures in relation to Adults.
A good standard of general education.
A recognised Degree/Diploma or equivalent qualification in Social Work i.e. Degree, DipSW, CQSW etc.
Registration with Social Care Wales.

Responsibilities

Undertake assessments for care and support, including CHC assessments.
Work with individuals whose needs may be complex or require ongoing monitoring and support.
Collaborate with colleagues to ensure wellbeing outcomes for service users.
