Wokingham Borough Council is seeking a qualified Social Worker to join the All Age Continuing Care and S117 Support Team. This full-time fixed-term role supports adults and carers, with training provided to develop within the specialist area.

Joining a supportive, award-winning authority, you will work alongside health and social care partners to secure funding, coordinate assessments and deliver outcomes aligned with Care Act duties.

Qualifications

• Qualified Social Worker registered with Social Work England.
• Good understanding of social care law and professional practice.
• Full Driving Licence and Daily Access to a Car.
• Solution-focused, compassionate team player.

Responsibilities

• Assessing and reviewing the needs of adults and carers in line with the Care Act 2014.
• Offering advice, guidance, and access to the right support services.
• Carrying out risk assessments and developing safety plans.
• Working in a person-centred way, respecting people's dignity, culture, and wishes.
• Using a strengths-based approach to help people live independently.
• Working closely with carers, families, and partner organisations.
