West Sussex County Council is seeking an experienced Social Worker for the Discharge Outreach Recovery Service. The role combines community-based assessments with hospital social work duties, supporting safe discharges and wellbeing.

The post is full-time (37 hours), hybrid with Worthing office base, and requires flexible hours and a valid driving licence. Salary is £41,177 to £48,738 per annum, with a fixed-term contract to 31 March 2027 and internal progression routes.

Qualificações

• Experience of proactive team-based strengths-based assessments in complex cases.
• Demonstrated leadership in partnership with carers and agencies.
• Experience with risk enablement and evidence-based practice.
• Ability to interpret complex information to prioritise needs.
• Experience leading safeguarding enquiries.
