Job summary

Westminster City Council is recruiting a Social Worker for the Hospital Discharge Team. Based at St Mary’s Hospital and serving Westminster and nearby boroughs, you’ll assess needs, develop discharge plans and work with health colleagues to support safe patient transfers.

You’ll undertake safeguarding and MCA assessments, promote person‑centred care, and maintain professional registrations, CPD, and DBS checks as required. Flexible patterns and collaborative teamwork are encouraged.

Qualifications

• CQSW or Diploma in Social Work or equivalent degree.
• Registered with Social Work England.
• Experience in Local Authority or health social care settings.
• Knowledge of relevant legislation and personalisation principles.

Responsibilities

• Undertake needs assessments and develop discharge plans.
• Conduct safeguarding and Mental Capacity Act assessments.
• Work with carers and other professionals to manage risk.
• Provide guidance to colleagues and participate in duty systems.
