Job summary

Caritas Recruitment is seeking an Advanced Social Care Practitioner to join our team in Nottinghamshire. The role focuses on adult community services, conducting assessments, developing care plans, and working with service users, families, and multidisciplinary teams to promote independence and wellbeing.

You will provide expert guidance to colleagues, manage complex cases, ensure compliance with legislation and safeguarding, and help users access community resources.

Qualifications

• Recognised social work qualification (Degree in Social Work, DipSW, or equivalent).
• Current registration with Social Work England.
• Significant post-qualification experience in adult social care.
• Strong knowledge of legislation, safeguarding procedures, and best practices.
• Excellent communication, interpersonal, and problem-solving skills.

Responsibilities

• Conduct comprehensive assessments of individuals' needs and risks.
• Develop and implement personalised care plans in partnership with service users and their families.
• Provide expert guidance to less experienced colleagues and contribute to their development.
• Manage complex cases and ensure compliance with relevant legislation and policies.
• Facilitate access to community resources and support services to improve quality of life.
